Request for Benefits Form

  • Once your schedule is final you may complete the FSU Request for Benefits form.
  • Check to be sure all areas have been addressed correctly before submitting.
  • Once you have submitted the form you will receive a confirmation email to your FSU email account stating the request form has been received by the FSU Veterans Benefits Office. Please keep this email for your records.
  • Any additional documents must be sent to the FSU Veterans Benefits Office in order to complete the process.
  • You must complete a new FSU Request for Benefits form, after registering for classes, for each semester you plan to use VA Educational Benefits.
