Once your schedule is final you may complete the FSU Request for Benefits form.
Check to be sure all areas have been addressed correctly before submitting.
Once you have submitted the form you will receive a confirmation email to your FSU email account stating the request form has been received by the FSU Veterans Benefits Office. Please keep this email for your records.
Any additional documents must be sent to the FSU Veterans Benefits Office in order to complete the process.
You must complete a new FSU Request for Benefits form, after registering for classes, for each semester you plan to use VA Educational Benefits.
